If You Know Who You Want to Work With:
1. Go back to our Meet Your Team page. Click the "Request Intake Session" button.
2. You'll be directed to our TherapyPortal.  If you  don't already have an account with us, please go to "Continue Without Signing In" to provide us your contact information and request an appointment. 
3. We will then email you a portal link invitation so you can fill out the six documents waiting there for you (in the Documents tab). 
4. When you review and sign those documents, your appointment will be scheduled and confirmed if it is still available. We operate on a first-come first-serve basis. The forms take about 10 minutes to complete.
