1. Go back to the Meet Your Team page. Click the "Request Intake Session" button.
2. You'll be directed to our TherapyPortal to request an appointment. We will hold that spot for 48 hours.
3. We'll send you six forms. They take about 10 minutes to complete.
4. When we see you've filled out the forms, we will confirm your appointment.
