If You Already Know Who You Want to Work With:
1. Go back to the Meet Your Team page. Click the "Request Intake Session" button.
2. You'll be directed to our TherapyPortal to request an appointment. 
3. We will then email you a portal link invitation so you set up an account, log in, and fill out the six intake documents (in the Documents tab). 
4. We will hold your appointment slot for 48 hours. Please fill out the forms within that timeframe and we will confirm your appointment. The forms take about 10 minutes to complete.
